Case Study: Healthcare Provider

Patient 360 Implementation

Working with a large Regional Healthcare provider, axsysTek helped transform the patient experience and back office for the Gastro-Intestinal care area.

Business Challenge 

A Provider with over 200 primary care and specialty physician practices, children’s specialty clinics, home health care, behavioral health, occupational health, rehabilitation services and urgent care facilities wanted to improve care management, outreach and back office processes for Gastro-Intestinal patients.

Solution 

Analyzed paper-based in-take, assessment, care plan and follow-up converting to Salesforce HealthCloud forms using MuleSoft for data exchange with Cerner and Epic EHR platforms. Patient profiles integrated into Salesforce Marketing Cloud to manage alerts, notifications, communications and interactions.

This new patient-centered framework enables the department to deliver more personalized, next-generation care improving how they manage and engage patients. System  empowers patients with more  on-demand health data and visualization tools so they can participate in their care journey.
